1. Apply for Request Letter of inquiry on personal conduct investigation at Dalian Notary Public Office.

2. Submit the Request Letter on personal conduct investigation and required documents to Dalian Public Security Sub-Bureau.

The Dalian Public Security Sub-Bureau checks whether the applicant has criminal records and issues the Dalian Police Certificate/Non-Criminal Record Certificate.

3.Submit the Police Certificate to Dalian Notary Public Office for notarization. Dalian Notary Public issues the Notarial Certificate of Criminal Record Check.

The applicant must provide the following documents

1. Scan copy of current and previous passport photo pages, the pages with China visa, the pages with Chinese residence permit and the pages with China exit & entry stamps.

2. Scan copy of registration form of temporary residence(TRRF).

3. Scan copy of the non-criminal background check application form.

4. Original photo. The photo should be in color and of the size of 2 inches * 2 inches.

5. If you cannot submit documents in person, you need to provide a Power of Attorney (POA) to nominate a representative to apply for Police Certificate/Non-Criminal Record Certificate on your behalf. Sample of POA is as follows:
